Position: Family Physician, Fees for Service Type: Full Time Location: Halton, ON Position Summary We invite you to join our dynamic and continuously evolving team at WELL Health as a Family Physician. This is an in‑clinic position, providing primary care services. You will support comprehensive patient care through the diagnosis, treatment, and management of acute, chronic, and preventive health needs. This position requires on‑site work at our clinic in Halton, Ontario. What you will be doing Provide full‑scope family medicine care within the 360° Care+ Program. Carry out all duties required of a primary care physician. Provide ongoing coverage when needed. Diagnose, treat, and manage urgent, acute, and chronic health concerns. Conduct preventive health assessments and ongoing wellness management. Order, interpret, and act on diagnostic and screening tests. Prescribe medications and therapeutic interventions. Perform minor procedures within scope. Collaborate with a multidisciplinary health‑care team to ensure cohesive patient management. Participate in team‑building activities when the schedule allows. Uphold all WELL Health core values (Team, Care, Growth & Integrity). Maintain accurate, timely patient documentation in the EMR. Participate in patient follow‑up, result communication, and care planning. Support program development and continuous improvement within 360 Degree Care. You have Licensed and in good standing with the College of Physicians and Surgeons of BC (CPSBC); membership with CMPA required. Strong clinical experience in primary care, including acute, chronic, and preventive health management. Experience or interest in lifestyle medicine, longevity medicine. Comfortable working in a collaborative, multidisciplinary environment. Excellent communication skills with patients and team members. Computer literacy and comfort with EMR systems. Confident, approachable, and committed to delivering an exceptional patient experience. One week of paid CME. Supportive leadership with ongoing coaching and feedback. Six weeks of unpaid vacation. Participation in WELL team and social events. Having a dedicated administrative team to support day‑to‑day operations.
