Developmental Services’ TAY home is a supported living program that provides for transitional aged youth (TAY) with developmental disabilities, supporting their transition from the child welfare system and assisting in developing identified life skills pertinent for their life stage. Our TAY Home offers a unique and innovative approach to skills training, providing an inclusive and supportive home where individuals can create and achieve their own unique goals. WAYS YOU CAN CONTRIBUTE Needs Analysis Collaborate with individuals with developmental disabilities to develop goals, goal plans and strategies that support those goals and enhance capacity to live independently. Conduct effective needs analysis to identify the unique needs of each individual. Service Delivery Support delivery of services and supports according to goal plans that foster an effective transition to independent living. Assist full‑time Senior DSWs in locating e‑programs, services, housing and employment opportunities that align with goal plans. Work effectively with other members of the TAY staff to develop strategies and implement therapeutic programming. Ensure appropriate personal and physical care, crisis response, counselling and emotional support are provided in an effective, trauma‑informed manner. Program Development Support development of programs within and outside the home and implement life‑skills facilitation that enable individuals to live and integrate into the community independently. Ensure program plans are developed and implemented to promote independent living, are based on individual needs/goals, reflect a high level of learning, and are planned in consultation with the Supervisor. Facilitate life‑skills programming, either one‑on‑one or in group settings, based on individual needs, goals and support plan, and within set timelines and standards. Participate in assessing the long‑term service needs of individuals and the program. Community Outings Provide supports for individuals to enable active participation in community activities and/or recreational outings in a safe manner. Plan community outings, appointments and activities in accordance with individual needs, safety protocols, available resources and programming; promote social inclusion. Crisis Management Reduce safety incidents through strategies that reinforce positive behaviour and identify issues before, during and after behaviour through accurate information collection as required by CPI and funder guidelines. Apply Individual’s Behaviour Support Plans and Safe Management consistently in crisis situations or when dealing with challenging behaviour. Program and Administrative Duties Prepare professional, clearly written materials and correspondence that reflect the agency philosophy and protect the privacy of individuals in service. Monitor finances for individuals as required by established policies and practices. WHO YOU ARE Education / Experience Post‑secondary diploma in Social Services and/or Developmental Services or a degree/diploma in a non‑related area with at least two years of experience in developmental services. Valid CPR/First Aid and Crisis Prevention Institute (CPI) certifications. Strong knowledge of computer applications (MS Word, Excel, PowerPoint, Outlook). Minimum two (2) years of recent work‑related experience in the following areas: working directly with individuals with a developmental disability or dual diagnosis; case management; supporting youth involved in the child welfare system; working from a trauma‑informed and harm‑reduction approach; engaging with diverse populations; applying a culturally responsive lens; understanding of the Social Inclusion Act and the Act for the Supports of Persons with Developmental Disabilities (2008); working knowledge of the Developmental Services Sector. Skills / Abilities Knowledge of the Developmental Services Sector. Proven individual and group facilitation skills that increase independence in activities of daily living. Assessment skills for delivery of service to individuals and daily operational/programming issues, including suitability and risk determination. Program planning skills to implement, evaluate and modify new and existing programs. Understanding of Trauma‑Informed Care practices. Effective crisis intervention skills to diffuse challenging situations; ability to work well under pressure to minimize service interruptions. Written communication skills in English; clear concise communication. Verbal communication skills in English; clear concise communication.
