Right Siting Officer, PHIC - Strategic Planning and Evaluation The Right Siting Officer is responsible for assisting with navigating patients to the appropriate downstream care services available in the community. They will work closely with physicians, nurses, administrators and actively liaise with community-based care providers to facilitate the transfer of patients from one care site to the next, based on patients’ care needs and social and financial circumstances. Responsibilities Provide assistance to clinicians by explaining the available options for follow-up in the community. Address clinicians’ queries about patients’ discharges and follow-ups. Assess patients’ needs and care requirements and recommend appropriate community-based services as appropriate. Brief patients on the available follow-up options in the community, including financial counselling. Gather feedback and conduct patient satisfaction surveys. Contact GP clinic/SingHealth Polyclinics (SHP) for booking of follow-up appointment. Prepare relevant documents (e.g. discharge summary) and raise referral to appropriate care services via respective referral platforms. Update patient records and maintain upkeep of data on counselled and right-sited patients. Participate in staff training and keep up with updates on community-based care services. Any other adhoc duties as assigned. Qualifications GCE O Level and above. Pleasant disposition and good communication skills, comfortable with managing difficult situations. IT savvy and proficient in using Microsoft Office; prior experience in office administration will be valuable.
