Job Description Position: Rapid Access Clinic for Low Back Pain (RAC-LBP) – a shared‑care model to reduce chronic low back pain, opioid initiation and unnecessary imaging and specialist referral. Location: Toronto Western Hospital, Department: Allied Health, Reports to: Clinical Manager. Status: Independent Contractor. Responsibilities Acts as the first and primary point of contact for consultation and assessment of referred patients with low back pain. Provides specialized care, education and counseling for patients. Performs a medical history, physical and health status assessment. Conducts comprehensive assessment and communicates a diagnosis. Determines the suitability and eligibility of patients for surgical assessment. Differentiates between inflammatory and non‑inflammatory conditions. Identifies signs and symptoms requiring diagnostic imaging or referral to a specialist per RAC‑LBP guidelines. Initiates communication and appropriate referrals based on guidelines. Establishes, communicates and documents clinical impressions/medical diagnoses within scope of practice. Develops a plan of care in collaboration with the patient/family and primary care provider. Documents the plan of care per guidelines. Communicates internally and externally to facilitate care. Reevaluates and revises the plan of care with patient/family and provider. Communicates specific medical diagnoses to the primary care provider. Utilizes a patient‑centered approach consistent with patient goals, needs and values. Qualifications At least 4 years of clinical experience following completion of an accredited entry‑to‑practice program in a regulated health profession. At least 1 year of clinical experience in the musculoskeletal‑spine area. Member in good standing of the respective professional/legislative college at the provincial level.
